home. Primary Care Physician Role Overview: The Primary Care
Physician provides comprehensive primary care services to a panel
of veterans, ensuring continuity of care within a multidisciplinary
healthcare team. The provider will be responsible for the
assessment and care of new and returning patients with any number
of medical problems (e.g., hypertension, diabetes, ischemic heart
disease, COPD, depression, chronic pain). The care may be delivered
via face-to-face, telehealth, or video visits though this position
may require occasional travel to VISN medical centers or
Community-Based Outreach Clinics (CBOCs). Responsible for ordering
diagnostics, consultations, medications, and other therapies as
medically indicated, meeting preventative health maintenance needs,
and completing all required VA computer documentation including
clinical reminders and screenings in a timely manner. Telehealth
and/or video visits may only be conducted from sites of care within
the VA Northern California Health Care System or VA-approved
alternate work sites. The provider may also support the Call Center
as well. Responsibilities Include: Veteran-Centered Care: Deliver
comprehensive primary care services with an emphasis on health
maintenance, disease prevention, patient education, and chronic
disease management. Promote shared decision-making with veterans
and their families to achieve optimal health outcomes. Patient
Aligned Care Team (PACT) Model: Implement and practice according to
the PACT model, which focuses on collaboration and coordination
among the multidisciplinary healthcare team members. Utilize
evidence-based practices to enhance patient care and outcomes.
Quality Improvement: Participate in continuous quality improvement
projects to enhance the delivery of care. Utilize various quality
measurement tools to monitor and improve clinical performance and
patient outcomes. Clinical Leadership: Serve as a mentor and role
model for other healthcare providers within the primary care team.
Engage in educational activities, providing guidance and support to
residents, students, and other trainees. Healthcare Coordination:
Coordinate care for veterans involving specialty care, mental
health services, and other ancillary services to ensure
comprehensive healthcare delivery. Collaborate with other
healthcare providers to develop individualized treatment plans for
veterans. Use of Technology: Utilize the VA's electronic medical
record system for documentation, ordering of tests, and
communication with patients and healthcare team members. Embrace
telehealth as an integral part of delivering primary care services.
