Physical Therapist Location: Oakland, CA Agency: Cynet Health Pay: $2,959 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position Job Title: Physical Therapist Profession: Rehabilitation Therapy Specialty: Physical Therapist Duration: 13 weeks Shift: Days Hours per Shift: 40 Experience: Minimum of 1 year License: CA PT License Certifications: None specified Must-Have: One year of Home Health experience Description: The role requires prioritizing start of care visits. Supervision of Physical Therapy Assistants is required. Next day mandatory reporting of start of care visits is essential. Clinical notes and other documentation are due within 48 hours of the visit. Timely communication with the supervisor, intake, and office is necessary. General responsiveness during normal business hours is expected. The territory may require expansion as needed. Schedule consists of 40 hours per week, Saturday through Wednesday. Training will consist of three full days, one being in-house and two in the field. The documentation system used is Kinnser. Equipment issued includes an iPad and Verizon One Talk. Mileage reimbursement is provided from the first to the last patient. The company car is available as an option with a decreased bill rate. The productivity requirement is set at 30 points per week. Caseloads and visit mixes per day will equal an average of 8 hours per day and 40 hours per week. Overtime must be pre-approved by the Clinical Supervisor. Visit types include Start of Care, Recertification, Resumption of Care, OASIS Discharge, Evaluation, Revisit, and NON OASIS DC.
