Licensed Social Worker Location: Mammoth Lakes, CA Agency: Cynet Health Pay: $1,908 per week Shift Information: Days - 3 days x 10 hours Contract Duration: 13 Weeks Start Date: 1/26/2026 About the Position Job Title: Clinical Social Worker Profession: Behavioral Health Specialty: Clinical Social Worker Duration: 13 weeks Shift: Day shifts Hours per Shift: 30 Experience: Minimum 2 years License: California Licensed Clinical Social Worker required Certifications: N/A Must-Have: Bachelor's in Social Work required, Master's degree preferred. Three to five years hospital social work experience preferred. Description: The Social Worker collaborates with the Case Manager to develop post-acute care plans for patients experiencing personal, social, or environmental barriers to wellness and recovery. The Social Worker intervenes with patients who have complex psychosocial needs. They provide assistance with eligibility determination for social programs and funding sources. The Social Worker coordinates community assistance from various agencies. They offer crisis intervention to patients and families with psychosocial needs. They facilitate the development of a discharge plan of care for high-risk patient populations. This position requires communication with patients and families to ensure an adequate continuing care plan is developed for discharge. The Social Worker assists patients and families in resolving personal and environmental difficulties that may predispose an individual to illness. They support planning and post-discharge processes for patients. The Social Worker screens, assesses, and provides resources regarding the specific social drivers of health needs for patients and families. They act as an expert in social drivers of health resources available in the community. Patients are referred to Behavioral Health for formal assessment needs and/or outpatient services as appropriate. The Social Worker provides education and support for food access and housing. They also coordinate the patient transportation program. They assist families in crisis situations in understanding the complexities of the patient's medical situation and its impact on lifestyle. The Social Worker conducts financial assessments and referral services. They arrange for in-home care and equipment as needed. Candidates should be compassionate, empathetic, and understanding. They should be highly motivated, creative, and resourceful. Exceptional communication skills are required. Candidates must be proactive problem identifiers and problem solvers.
